Process Safety Analysis : An Introduction by Bob Skelton (June 1998) (Link)
Shows how safety assurance is actually performed in industry and examines the various stages of safety analysis in the design and operation of process plant. Emphasizes the HAZOP technique and covers control of sources of ignition and maintenance procedure.

HAZOP and HAZAN : Identifying and Assessing Process Industry Hazards
by Trevor A. Kletz (April 1999) (Link)
This reference provides basics of applying hazard and
operability study (HAZOP) and hazard analysis (HAZAN) in the
process industries. Chapters cover hazard identification and
assessment, hazard and operability studies, real-life
accidents that could have been prevented by HAZOPs, hazard
analysis, and management issues. Other subjects are objections
to HAZOP and HAZAN, limitations on the application of
quantitative methods to railway travel, sources of data and
confidence limits, and the history of HAZOP and HAZAN. This
fourth edition is about 40% longer than the last edition, with
more examples of applications. The author is affiliated with
the Royal Academy of Engineering, the Royal Society of
Chemistry, and the American Institute of Chemical Engineers.

AcuTech,
Incorporating
Inherently Safer Design Practices into Process Hazard Analysis (.PDF,
45K)
Describes how PHA methods may be used to recognize potential
hazards, and weigh the likelihood of the event against potential
consequences. Such studies may be used to improve process design
practices and produce "inherently safer designs."
A
Risk Based Approach to Addressing Recommendations from PHA Studies
(.PDF,
52K)
This paper discusses an approach aimed at streamlining the
decision-making process and minimizing ambiguity in decisions. The
characteristics and benefits of improved prioritization processes will
be reviewed. Possible barriers to the implementation of improved
processes, and the future of risk-based prioritization of risk
reduction actions will be discussed.
PHA
Program Quality Self Examination (self examination of your PHA
technical approach and follow up) (.HTML,
12K)
Checklist provided to allow for a self examination of your PHA
technical approach and follow up.
Buying
PHA Software with Confidence (.PDF,
37K)
Describes the key points in purchasing PHA software, and offers a
step-by-step set of questions in making a decision.
Ensuring
Quality in PHA Studies (.PDF,
21K)
Describes the purpose of conducting PHAs, underlying issues that
affect their quality, and how to prepare a protocol to improve them.
The
Use of a Ranking Matrix and Recommendations Prioritization System for
Process Hazard Analysis Studies (.PDF,
25K)
In the absence of formal, approved risk management tools, it is
recommended that individual companies adopt a standard, defensible
ranking scheme to provide a common basis for decision-making, based on
the company’s defined safety goals. This paper discusses an
approach, common to many companies, that qualitatively ranks the risk
of identified scenarios. This paper also discusses the possible
problems of using various ranking schemes and risk criteria.
